Below are commonly used forms for my office. For your convenience and to save time, you can print, fill out these forms and bring them to your first appointment. Like other healthcare professionals, I am required by law to keep accurate, confidential records on all clients. I reserve the right to deny service to anyone who refuses to fill out the forms. If you forget the forms, I will make them available for you prior to the start of the session. Please arrive 15 minutes early to fill out this paperwork. If you have advanced disease, I may ask to speak with your primary care physician to coordinate your care. Please contact me if you have any questions or concerns.
